Healthcare professionals agree that health is more than what happens to a patient inside of the clinic or hospital. Patient health also includes the social determinants of health, or the social factors that impact an individual’s ability to achieve wellness and engage with the healthcare system.

Key social determinants of health, including transportation, affordable and stable housing, and nutritious food accessibility, each have a direct impact both on patient health and healthcare spending. As health plans work to deliver value-based, quality healthcare to its members, they may consider the social determinants of health and their own initiatives to address them.

Learning Objectives:

•Create plan for detecting major social needs or care gaps
